Medical Form (Type Online)
This is a form that you complete online and then print. If the form fill-in does not work on your computer the cause is the browser using it's own PDF viewer to display the document.
Solutions:
- Save the form to your computer. Then open Adobe Reader and use it to open the form.
- Try another browser.
- Print the form three times and fill out by hand.
